The life preserver offers itself to you at the end of every round. I think in the tutorial (Dr. Scuttles) it tells you that the life preserver helps you to secure a point bonus for every 1000 meters completed. Since it does not say anywhere what that bonus is, this creates little temptation to get the life preserver.
The cost of the life preserver is 15 sand dollars!!! That is a LOT of currency to spend on something that will extend one round. Further in its disfavour I will say that when a round is already so far along that lots of dangers appear and once and you are moving too fast to avoid them, the most that the round will go on is a few meters -- not worth the loss of 15 sand dollars.
Perhaps the least sensible thing about the life preserver is that it appears as a mission: use one life preserver. If the purpose of the mission is to earn enough stars to receive a sand dollar, and you need to earn 103 stars to get fifteen sand dollars [see mathematical calculations in the introduction to this forum] and using a life preserver gives you two stars, this is giving up fifteen sand dollars for the gift of earning 2% of one sand dollar (that is, you are giving up about 750 times what you are getting). It is MUCH better to buy out this mission for 10000 coins!
